Guillain-Barré (GBS)
è una rara neuropatia immuno-mediata, che colpisce persone
in buona salute. Lo spettro clinico della GBS è eterogeneo
e comprende la polineuropatia infiammatoria demielinizzante acuta
(AIDP), la neuropatia motoria assonale acuta (AMAN), la neuropatia
sensoriale assonale e motoria acuta (AMSAN) e la sindrome di
Miller-Fisher
(MFS). La malattia è caratterizzata da un esordio rapido,
con debolezza simmetrica agli arti, che progredisce per alcuni
giorni fino a 4 settimane e colpisce i pazienti di tutte le età.
La maggior parte dei pazienti presenta anche disturbi sensoriali (formicolio o ipoestesie). Nella maggior parte dei casi precede l'esordio della debolezza agli arti una malattia infettiva, soprattutto l'infenzione da Campylobacter jejuni. A causa del progredire della debolezza, circa il 25% dei pazienti necessita, per un po' di tempo, di respirazione artificiale. Con il tempo, migliora la forza muscolare e diminuiscono i disturbi sensoriali. Molti pazienti restano invalidi per mesi o persino per anni. La stanchezza e l'intolleranza alla fatica possono durare per anni. Una buona ospedalizzazione in un reparto di terapia intensiva è essenziale. Si è dimostrato efficace il trattamento con immunoglobuline per via endovenosa o la trasfusione di sangue. La fisioterapia e la riabilitazione sono importanti. Si pensa che la risposta immune contro gli agenti infettivi scatenanti sia coinvolta nella patogenesi della GBS, attraverso una reazione crociata con i tessuti neurali. Dott. Overview Guillain-Barré syndrome (GBS) is an inflammatory disorder of the peripheral nerves. The peripheral nerves convey sensory information (e.g., pain, temperature) from the body to the brain and motor (i.e., movement) signals from the brain to the body. GBS is characterized by weakness and numbness or tingling in the legs and arms, and possible loss of movement and feeling in the legs, arms, upper body, and face. Chronic inflammatory demyelinating polyradicalneuropathy (CIDP), is considered to be a related form of Guillain-Barre syndrome. It is much less common than GBS, and evolves much more slowly and usually is longer lasting. Some CIDP patients experience periods of worsening and improvement, and individual relapses are often confused with GBS. Incidence Guillain-Barré
syndrome
is
a
rare
disorder;
its
frequency
is
about 1 to 2 cases in every 100,000 people per year. Men and
women, young and old, are equally prone to contracting GBS.
